Use this form only for warranty claims, returns, or exchanges. Please fill out all sections accurately to avoid delays.

Submit a separate form for each product being returned. TELLUS does not cover return shipping unless approved.

After completing the form, include a printed copy inside your return package. If you’re requesting a prepaid return label, wait for an email with the label before sending your package.

Prepaid Shipping Label
Prepaid shipping labels are issued by TELLUS to the costumer on a case-by-case basis.
Please provide the desired address where you want your replacement(s) / exchange(s) sent. If you do not provide shipping instructions, we will ship the order to the last known address we have for you with ground service.
